Graham will don the famous red, white and blue strip for the 34th and final time this weekend after signing a one year deal with the Titans.
"I have really enjoyed my time here at the Chooks, everyone that works at the club has been great to me and really made me feel welcome since the day I first walked in the door," Graham said
"I have learnt a lot under Smithy and the entire coaching staff and I was hoping to stay on at the club but it wasn't to be."
Despite playing a starring role in the push to last year's grand final, Graham cruelly missed out on the decider due to a pectoral muscle suffered in a qualifying final against the Panthers.
"It was disappointing to miss out on the big one but the whole year last year was great," he said
"There were a number of ups and downs throughout the season but the way we finished it and the win against the Tigers is something I will never forget."
Like most of the more experienced players in the squad, Graham who played 111 matches for the Raiders before arriving at the Tricolours, is excited by the batch of young talent coming through the club.
"All these young blokes in the team at the moment are so talented and with the coaches we have here, they can make absolutely anything of themselves."
An undeniable fact that should have Roosters fans excited for the possibilities in the years ahead.
So it's next stop Gold Coast for the likeable Graham but there are a few things on his radar before the move up north.
"We have a big game to get through this weekend and it will be a tough game against the Storm but the boys have really been putting in and we are confident we can end the season on a high."
A trip to Melbourne and a possible overseas holiday are then on the cards for Graham before he settles in to his new digs in late October.
"I am excited about the move; there will be plenty of new faces up there so will be an exciting time to be involved."
Unsure about whether it will be his last season, Graham is happy to just play it by ear.
"I haven't really given retirement much thought, I will just see how my body pulls up next year but at the moment I feel like I still have plenty to offer."

Source: www.titans.com.auThe Jetstar Gold Coast have added further depth to their backline for next season with the signing of speedy centre/winger Phil Graham on a one-year contract for season 2012.
Graham has made a name for himself as a prolific try scorer in the NRL, having notched 79 tries in 142 NRL matches throughout an 11-year career with the Canberra Raiders (2002-09) and Sydney Roosters (2010-11).
The 30-year-old will join incoming centres Jamal Idris (Bulldogs) and Beau Champion (Melbourne) in competing for spots in the outside backs next season along with the likes of existing flyers David Mead, Kevin Gordon, Steve Michaels and William Zillman.
Jetstar Titans Managing Director Michael Searle said Graham would add further versatility to a backline which has already shown its utility value this season with examples such as Zillman, who has played at fullback, five-eighth and in the centres at different times.
"With Preston retiring, we had the option to look at a speed player that could add some support in the outside backs and we felt that Phil was a suitable replacement given he can play fullback, wing and in the centres,'' said Searle.
"He's played well for the Roosters despite limited opportunity in the top grade this season and he's at that age where we are confident he can add value to our squad both on and off the field.''
Jetstar Titans Head Coach John Cartwright said Graham's experience, height and speed would make him a welcomed addition to the playing squad for next season.
"He's got plenty of experience and height about him which is something that we've lacked,'' said Cartwright of the 186cm Graham.
"He plays centre, wing and fullback so he's going to help out our young blokes and put pressure on for spots.''
Graham, who hails from Tamworth in NSW, said he was excited about the fresh start next season having been hindered by injury at the start of this year after suffering a torn pectoral muscle in last year's finals series.
"It seems like a really good club from the success the Titans had last year,'' said Graham.
"With the new players that are coming in next season and the people that are already there, it seems like a really talented bunch. I'm just looking forward to getting up there.''
